primary services are offered at one or more of the Vincentian Collaborative
Services' sites.
Skilled Care A
level of care that must be given or managed by licensed health care
professionals and is under the general direction of a doctor
Intermediate Care Health-related
care provided under a "plan of care" not requiring "skilled
care"
Long Term Care Care
provided for an extended period
Short Term Care A
limited period of care until a recovery progresses far enough for
independent living
Personal Care 24-hour
supervision in a residential setting
Memory Deficit Care Care
dealing with the special needs of those who are cognitively impaired
(such as Alzheimer's patients)
Therapy Physical,
speech, and occupational therapies to rehabilitate and restore independence,
available six days per week
Family Hospice Care Support
and counseling services for family members
Pastoral
Care The
opportunity for worship, prayer, emotional support, spiritual growth,
and faith development, regardless of religious affiliation
Assisted Living Care
that provides a high level of independence, for which only limited
assistance is needed with tasks of daily living
